Assessment 3 Instructions: Assessing the Problem: Technology, Care Coordination, and Community Resources Considerations
The World Health Organization recognizes hypertension as one of the major risk factors for mortality and morbidity accounting for approximately nine million deaths annually. Primary hypertension silently affects 73.6 million Americans(Zhang & Moran, 2017). The sequelae of uncontrolled blood pressure is end-organ damage, which results to stroke heart failure, kidney failure, and heart attack. Notwithstanding, hypertension is among the most preventable causes of death and end organ complications. Blood pressure management through lifestyle modification and antihypertensive medication have indicated efficacy in reducing the risk factors for subsequent cardiovascular complications(Zhang & Moran, 2017). However, regardless of the clear benefits of blood pressure management, a substantial part of the population of diagnosed and undiagnosed cases are not receiving adequate care. To tackle the hypertension burden on the health system comprehensively, traditional health models of managing hypertension including office visit must augment their approaches with the modern approach that relies of new technologies to facilitate patient’s self-management to manage blood pressure effectively. E-Health refers to the broad range of activities that utilize communication and information technologies (ICT) to store, retrieve, and distribute health related information for administrative, prevention, diagnosis, treatment, monitoring, and educational purposes. The last two decades have witnessed rapid development of technology consequently enlarging the scope of digital health processes that involve internet use to deliver care over a distance(Silva et al., 2015). The main categories of e-Health include EHRs, grid technologies, clinical decision support tools, remote diagnosis and teleconsultation, medical research, and telehealth. The major applications of e-Health include on-line medical records, e learning, medical imaging and diagnostic, interactive systems, wearables, and online prescription
